Blue Light May Treat ED: Using Gene Therapy to Produce Effective Erections

  When facing erectile dysfunction, many men turn to the "little blue pill," but Viagra can actually only prolong an erection, not initiate one. Researchers have now found a new method using gene therapy to produce effective erections.
  From age 30 onwards, the number of men with abnormal erections has increased. Among men over 60, more than half have been affected by erectile dysfunction. Main causes include cardiovascular disease, diabetes, endocrine disorders, neurological diseases, and drug side effects; even spinal cord paralysis can render a patient unable to achieve an erection.
  In this new therapy, a gene vector that responds to blue light is injected into the erectile tissue of the penis. Once exposed to blue light, a precursor molecule (guanosine triphosphate, GTP) is converted into the second messenger cyclic guanosine monophosphate (cGMP), which is present in many organs of the human body. This process closes voltage-dependent calcium channels, thereby reducing the intracellular calcium concentration; the muscle fibers relax, allowing more blood to flow into the erectile tissue and resulting in a normal erection.
Afterwards, an enzyme degrades cGMP, and the erection gradually subsides. Viagra enhances and prolongs erections by inhibiting the activity of this enzyme, but it cannot trigger an erection. With this gene carrier, cGMP production does not come from sexual arousal but requires exposing the erectile tissue to blue light.
Martin Fussenegger, the biotechnology professor at ETH Zurich who led the research team, said: "This way, we can leverage the principles of normal sexual stimulation to trigger a cascade of signaling responses in the body, ultimately achieving an erection."
The researchers tested this method on male rats with very good results. In most cases, the blue light controlled the rats' erections like a switch. For some rats, blue light stimulation even triggered ejaculation. Fussenegger said: "The erectile system is very similar across all mammals, and Viagra also works on rats, with effects comparable to those in humans." Therefore, he believes this method could also work for humans.
Fussenegger stated: "Injection should not be a barrier to use, because injecting drugs into the erectile tissue is already a common practice for treating erectile dysfunction." This tissue has essentially no pain sensation and is largely separated from normal blood circulation, so the gene carrier is unlikely to reach other parts of the body. In addition, cGMP degrades relatively quickly, and since Viagra can prolong erection, it can serve as a supplement to gene therapy.
Fussenegger believes that artificially induced erections can bring great satisfaction to patients with erectile dysfunction: "Several doctors have already confirmed this to me." Additionally, not all patients are suitable candidates for Viagra, such as those with heart disease.
  Researchers spent 4 years developing this gene carrier, which is still only a preliminary model and has not yet been tested in humans. Fussenegger said: "Before it can become a treatment method, it must go through expensive clinical trials. We are doing our best to find partners, hoping to apply our technology in clinical treatment."
